  1. Some Companies promote people who get the job done quicker, unwittingly encouraging workers to ignore policies and procedures and take short cuts.
  2. Other reasons for employees taking short cuts include the following: a) to please their supervisor b) to gain acceptance c) to be viewed as a good worker d) job security e) job promotion.
  3. Some employees carry out tasks slower because they implement the appropriate safety procedures and are considered less than those who perform the tasks quicker without the procedures being in place.
  4. Some Companies forget to thank people for carrying out their tasks safely, and focus more on rewarding those who perform quickest.
  5. Many Companies blame people in their incident investigation process rather than look at the system failures with the aim to improve and prevent the hazards from causing harm again.
  6. Many behavioural Safety Programmes put people under scrutiny rather than looking into why people do what they do.
  7. Many people suffer from a less than favourable Safety Attitude, Risk Perception or Risk Tolerance due to a lack of understanding and training.
  8. Most Companies don’t use Human Factors or Human Failure Types during the Risk Assessment procedure or Incident Investigation.
